These are all forms you'll need after you have completed Registration! If you still need to register, head to the Registration page. The forms on this page will be completed before your child starts school in the fall, and some are for reference while they're here.
Immunization Records
As of July 2026, we only need a copy of a student's immunizations on file, and not the previously used medical form. The qualifications for the immunization list are as follows:
The dated immunization record is to contain the following information:
(a) The child's name and birth date.
(b) A record of the immunizations that the child has had, specifying the month, day and year of each immunization.
Enrollment + Health Info
This is a state required form that must be submitted by the Parent Orientation date. A paper copy can be obtained from the school, or printed by following the link below, and return completed to the school. Per the state, every single area must be filled out, even if it's just N/A.
Tuition Breakdown
If you are interested in paying the full year or 1/2 year, please email us at info.cbcp@gmail.com and we will give you information on payment methods, as only monthly payments can be made online. These payments must be made by June 1/ Orientation. May’s tuition is due by Orientation Day each year. September’s tuition is due by the first week of school each year.
